Volunteer Release and Waiver of Liability Form
The undersigned volunteer (“Volunteer”) is at least 18 years old and desires to provide volunteer services for Food For Free Committee, Inc. (“Food For Free”) and engage in activities related to serving as a volunteer and agrees to do so in accordance with the following terms.
1. Scope: Volunteer understands that the scope of Volunteer’s relationship with Food For Free is limited to a volunteer position and that no compensation is expected in return for services provided by Volunteer. Food For Free will not provide any benefits traditionally associated with employment to Volunteer. Volunteer is responsible for their own insurance coverage in the event of personal injury or illness as a result of providing services to Food For Free.
2. Waiver and Release: Volunteer releases and forever discharges and holds harmless Food For Free and its successors and assigns from any and all liability, claims, and demands of whatever kind of nature, either in law or in equity, which arise or may hereafter arise from the services provided to Food For Free. Volunteer understands and acknowledges that Volunteer is discharging Food For Free from any liability and waiving any claim that Volunteer may have against Food For Free, with respect to bodily injury, personal injury, illness, death, or property damage that may result from providing services to Food For Free.
3. Medical Treatment: Volunteer grants Food For Free permission to authorize any necessary emergency medical treatment. Volunteer releases and forever discharges Food For Free from any claim whatsoever which arises on account of any first-aid treatment or other medical treatment rendered.
4. Acknowledgment of Risk: Volunteer acknowledges that the services provided to Food For Free may include activities that may be hazardous or involve risk. Volunteer knows of no reason, medical or otherwise, that would prevent participation in these activities and other volunteer services.
5. Photographic Release: I grant and convey to Food For Free all right, title, and interests in any and all photographs, images, video, or audio recordings of me or my likeness or voice made by Food For Free in connection with my providing volunteer services to Food For Free.
6. Other: This document will be governed by and interpreted in accordance with the laws of the State of Massachusetts, and is intended to be interpreted as broadly as possible. In the event that any clause or provision of this document is deemed invalid, the enforceability of the remaining provisions Release will not be affected.
By signing this document, I express my understanding and intent to enter into this Release and Waiver of Liability willingly and voluntarily.

Volunteers 15-17 years old must have a parent/guardian sign the Minor Volunteer Consent, Release and Waiver of Liability Form.
Volunteers 10-14 years old must have a parent/guardian sign the Minor Volunteer Consent, Release and Waiver of Liability Form AND must be accompanied and supervised by their parent or guardian at all times during volunteer shifts.
Nobody under 10 years old may volunteer at Food For Free.
Minor Volunteer Consent, Release and Waiver of Liability Form
The undersigned is the parent/guardian (“Parent”) of the minor individual identified below (“Minor”) who is between 10 and 17 years old and desires to provide volunteer services for Food For Free Committee, Inc. (“Food For Free”) and engage in activities related to serving as a volunteer. Parent consents to Minor doing so in accordance with the following terms.
1. Scope: Parent understands that the scope of Minor’s relationship with Food For Free is limited to a volunteer position and that no compensation is expected in return for services provided by Minor. Food For Free will not provide Minor with any benefits traditionally associated with employment. Parent is responsible for their own insurance coverage in the event of personal injury or illness as a result of Minor providing services to Food For Free.
2. Waiver and Release: Parent, on behalf of themselves and Minor, releases and forever discharges and holds harmless Food For Free and its successors and assigns from any and all liability, claims, and demands of whatever kind of nature, either in law or in equity, which arise or may hereafter arise from services provided by Minor to Food For Free. Parent understands and acknowledges that Parent is discharging Food For Free from any liability and waiving any claim that either Parent or Minor may have against Food For Free, with respect to bodily injury, personal injury, illness, death, or property damage that may result from providing services to Food For Free.
3. Medical Treatment: Parent grants Food For Free permission to authorize any necessary emergency medical treatment to Minor. Parent releases and forever discharges Food For Free from any claim whatsoever which arises on account of any first-aid treatment or other medical treatment rendered.
4. Acknowledgment of Risk: Parent understands that verbal instructions will be given to minor volunteers regarding the services to be performed and agree that Minor is able to follow any and all safety and other instructions. Parent acknowledges that the services provided to Food For Free may include activities that may be hazardous or involve risk. Parent knows of no reason, medical or otherwise, that would prevent Minor’s participation in these activities and other volunteer services.
5. Photographic Release: Parent grants and conveys to Food For Free all right, title, and interests in any and all photographs, images, video, or audio recordings of Minor or Minor’s likeness or voice made by Food For Free in connection with Minor providing volunteer services to Food For Free.
6. Other: This document will be governed by and interpreted in accordance with the laws of the State of Massachusetts, and is intended to be interpreted as broadly as possible. In the event that any clause or provision of this document is deemed invalid, the enforceability of the remaining provisions Release will not be affected.
